Job Description:

We are seeking a highly motivated and compassionate individual to provide support and promote the individual needs, skills, abilities, and personal goals of people with disabilities.

The successful candidate will have a strong commitment to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities.

Main Responsibilities:
  • Support individuals living in their own homes or in Supported Independent Living in their day-to-day living, ensuring that they have maximum opportunity for developing valued and independent lifestyles.
  • Carry out designated duties/tasks as per the service duty roster, and other reasonable duties as instructed by the employer or team leader.
  • Provide support to individual clients as required, including personal care and support at home and in the community, provision of domestic assistance, and support to access the community and participate in activities of their choice.
  • Facilitate community inclusion and participation, and support clients to access community activities, venues and services.
  • Encourage positive and socially appropriate behaviour, and effectively manage challenging behaviour where appropriate.
  • Maintain appropriate client records and provide progress reports as per established practices and procedures.
  • Participate in developing and monitoring individual program plans and be accountable for their outcomes.
Required Skills and Qualifications:
  • Certificate III in Individualised or Disability Services, or similar, or willingness to complete this within first 12 months of employment.
  • Minimum 12 months recent experience in Disability Support Worker or Aged Care role or being a Carer of a person with a disability.
  • A demonstrated commitment to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities.
  • Ability to develop positive relationships with service participants; problem solving and conflict management skills.
  • Ability to maintain privacy and confidentiality.
  • Good oral and written communication skills.
  • Demonstrated reliability, flexibility, and a positive attitude.
  • Commitment to values such as respect, empathy, and compassion.
  • Current Senior First Aid Certificate, unrestricted Driver Licence, and access to a reliable fully insured vehicle.
  • Working with Children Check, Police Check and NDIS Induction Training Certificate.
