Responsibilities:
- Work autonomously and collaboratively in an advanced nursing clinical role, providing high-level care and cultural connection for Aboriginal and Torres Strait Islander patients.
- Support the development of best practice emergency/inpatient care plans, and coordinate discharge plans for high-risk patients.
- Cultivate culturally safe partnerships with priority population groups, including Aboriginal and Torres Strait Islander people and culturally and linguistically diverse groups.
- Collaborate with internal and external community agencies and services to ensure seamless care delivery.
