Responsibilities include:

  • Provide personalized care that is genuine, compassionate and tailored to individual needs
  • Implement care plans developed by the Care Designer (Registered Nurse)
  • Meet client safety requirements and use equipment safely and effectively
  • Complete accurate and thorough records and documentation
  • Conduct nursing assessments

The ideal candidate will have:

  • Community nursing experience
  • Subacute and disability experience
  • Ability to identify and report client care concerns to the Director of Clinical Operations
  • Capacity to redirect or deescalate behaviors
  • Diabetes management skills
  • Dementia experience would be an asset
  • Leadership experience is an asset
