We are seeking a skilled and compassionate ACAT Assessor to join our team at the Northern Adelaide Local Health Network. As an ACAT Assessor, you will play a critical role in assessing clients' eligibility for Commonwealth-funded aged care services, working closely with carers, health professionals, and service providers to ensure the best possible outcomes for our clients.
Key Responsibilities- Conduct comprehensive assessments to determine client eligibility for aged care services
- Liaise with carers, health professionals, and service providers to integrate clinical experience and knowledge of aged care resources
- Participate in evaluation, administration, education, and quality assurance activities to ensure legislative requirements are met
To be successful in this role, you will need to be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold a current practicing certificate. Alternatively, you may hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines: Occupational Therapy, Physiotherapy, or Social Work.
