A Community Support Specialist
Job Description:This role is focused on empowering individuals to maintain their independence and well-being. You will conduct assessments with clients in their communities, using a standardized national assessment process that focuses on wellness.
You will work closely with each client to develop personalized support plans, tailored to their unique needs. You will also match and refer clients to relevant services, aiming to enhance their quality of life.
