As a Clinical Flow Coordinator, you will be responsible for managing inbound referrals of aged clients referred to Central Adelaide ACAT for a comprehensive assessment. Your primary objective will be to undertake intake/triage of inbound referrals using the National Screening and Assessment Form (NSAF) and the My Aged Care client record. You will enter accurate data from the intake/triage process into the My Aged Care system to determine appropriateness for assessment and clinical priority.
The Clinical Flow Coordinator is responsible for providing up-to-date information on appropriate aged care pathways that the client may be eligible to access. These include, but are not limited to, community, permanent residential, residential respite, and flexible care options, as well as State and Commonwealth-funded pathways available under the Aged Care Act 1997.
You will liaise with clients, carers, other health professionals, service providers, and other internal and external stakeholders. As a Clinical Flow Coordinator, you will ensure that you work within the Aged Care Assessment Programme guidelines and meet benchmarks and Key Performance Indicators (KPIs), as set out by the Commonwealth government and the Office for Ageing Well (OFAW) – SA Health.
You will participate in education, administration, evaluation, and quality assurance activities and ensure the legislative and funding requirements of the Department of Health and Ageing are met.
Key Responsibilities:
- Screening, managing incoming referrals, and conducting Support Plan Reviews (SPRs)
- Delivering information surrounding available aged care pathways
- Liaising with clients, carers, health professionals, and service providers
- Participating in education, administration, evaluation, and quality assurance activities
Requirements:
- Current National Police Clearance (NPC)
- Aged Care Screening check
- Immunisation screening as indicated in the role description
