Job Summary:

Evolve Therapy Services is a leading provider of therapy management services seeking a compassionate and skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in delivering high-quality care to our residents in a skilled nursing facility setting.

Key Responsibilities:

  1. Conduct Resident Assessments: Conduct regular screenings of residents to determine the best intervention or treatment for each individual patient.
  2. Develop Treatment Plans: Evaluate residents to inform and develop treatment plans, establish short- and long-term goals, and methods to achieve those goals.
  3. Communicate with Interdisciplinary Team: Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
  4. Establish Therapy Schedules: Establish individual resident therapy schedules in cooperation with other team members.
  5. Deliver Therapy Services: Conduct therapy according to established treatment plans and ensure proper documentation and billing of occupational therapy services.
  6. Oversee COTA: Oversee Certified Occupational Therapy Assistants (COTA) as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Requirements:

  1. Education: Degree in Occupational Therapy.
  2. Licensure: Active Occupational Therapy license in practicing state(s).
  3. Continuing Education: Ongoing continuing education and professional development to maintain current licensure and certification.
