Disability Case Manager

We are seeking a passionate Disability Case Manager to join our team in Bendigo.
The successful candidate will be responsible for creating and maintaining relationships between participants and their support teams, assessing participant needs, and developing individualized support programs.
About the Role- Lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.
- Report on the progress of clients, as it relates to the support provided, and prepare submissions for plan reviews as required.
- Coordinate staff rosters, resolve issues, and ensure compliance with relevant policies and procedures.
Requirements include proven experience working in the disability sector, excellent communication and interpersonal skills, and a professional level of administrative and business communication skills. A strong understanding of the NDIS is also essential.
Key Requirements:- Proven experience working in a supervisory or managerial role within the disability sector.
- Professional level of administrative and business communication skills, including case noting and report writing.
- Excellent time management and organizational skills.
- Strong interpersonal skills and high levels of resilience.
- Understanding of the NDIS and Microsoft Office suite.
