Job Summary

We are seeking a qualified Drug and Alcohol Clinician/Case Worker to provide integrated post-discharge outreach and care coordination for clients exiting the Involuntary Drug and Alcohol Treatment (IDAT) program.

Key Responsibilities
  • Provide assertive outreach case management for clients using drugs and/or alcohol.
  • Refer to inpatient/outpatient drug and alcohol treatment; undertake home visits; transport clients to medical appointments; assist with housing and income support; NDIS applications; referrals for community-based supports; liaise with medical teams; case coordination.
Required Qualifications
  • Current AHPRA registration for relevant discipline (Psychology, Occupational Therapy, or Counselling) with current registration in Counsellors Association of Australia (PACFA), or eligibility for Membership for Social Workers with AASW. For Health Education Officer, relevant health promotion/public health qualifications.
  • Demonstrated experience and commitment to professional and ethical boundaries in a medical multi-disciplinary treatment team.
  • Knowledge of the social and emotional needs of patients with drug and alcohol issues including trauma, grief and loss, domestic and family violence, child protection.
