Service Coordinator Job Description

As a Service Coordinator, you will act as the primary liaison between our organization and participants. This role involves empowering participants to live meaningful lives through effective communication, goal identification, and service coordination.

Key Requirements:

  • Build Relationships: Develop strong relationships with participants, families, and a diverse range of internal and external stakeholders.
  • Meet Service Level Agreements: Ensure service level agreements are consistently met and exceeded, maximising participant satisfaction.
  • Coordinate Intake and Exit Processes: Maintain accurate, comprehensive documentation and records to support participant care.
