Job Description:

We are seeking a highly skilled and dedicated Clinical Care Coordinator to join our team at Infinite Aged Care. The successful candidate will be responsible for ensuring that all residents within our facilities receive the highest quality of clinical care.

Key Responsibilities:
  • Lead clinical expertise: Provide guidance and leadership to clinical and care service teams across the facility.
  • Clinical needs management: Assist in leading carer teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
  • AN-ACC compliance: Manage all aspects of assessment, documentation, and submission of AN-ACC requirements within the facility.
  • Policies and procedures: Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
  • Care planning and review: Coordinate assessment, care planning, and review processes within the facility.
  • New resident admissions: Support the admission of new residents to facilities for optimal care assessments and initial consultations with residents and families.
  • Data accuracy: Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records, and staff time recording).
  • Roster coordination: Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
  • Clinical risk management: Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
