Experienced Nurse in Neurological Care

Our team is seeking an experienced nurse to provide high-quality care and support to individuals with neurological conditions. As a Senior Neurological Liaison Nurse, you will work closely with clients, their families, and our healthcare professionals to develop personalized care plans and deliver essential nursing services.
About the Role- Conduct comprehensive nursing assessments and create tailored care plans for clients with neurological conditions.
- Provide continence management, wound care, and palliative support to ensure optimal patient outcomes.
- Collaborate with healthcare teams to assess, advise, educate, and support clients and their families/carers.
- Manage workload and commitments of each MSWA Neurological Liaison Nurse by coordinating internal nursing referrals.
- Travel to client homes across the region, promoting independence and improving quality of life.
- Demonstrated time management and prioritization skills to handle a varied caseload.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- A minimum of 2 years of nursing experience, preferably in community or rehabilitation settings.
- Promote your career in a dynamic and supportive environment.
- Enjoy a mix of office-based work and community travel, making a tangible difference in clients' lives.
- Pool cars or reimbursement for your own vehicle will facilitate your travel.
- Current registration with the Nurses Board of Western Australia (as an RN).
- Current First Aid and CPR certification.
- NDIS Worker Screening Check (or willingness to obtain).
- Full C Class driving licence without restrictions.
- Working with children check.
- Experience in community nursing, rehabilitation nursing, or palliative care.
- Familiarity with neurological conditions, disability legislation, and the National Disability Insurance Scheme.
- Strong communication skills, both verbal and written, alongside computer literacy.
- Problem-solving abilities and a commitment to quality improvement.
