Job Title: Case Coordinator
Job Summary:
The Case Coordinator role at Tradewind Australia involves delivering high-quality case coordination services to clients. This position requires creating and implementing individualised case plans that address client needs, promote independence, and ensure ongoing safety and wellbeing.
Key Responsibilities:
- Develop and implement case plans based on client assessments.
- Provide culturally appropriate support to clients from diverse backgrounds.
- Ensure timely and accurate completion of incident reports, risk assessments, and safety plans.
- Refer clients to appropriate support services and maintain up-to-date records.
- Collaborate with internal teams and external stakeholders to meet case plan objectives.
- Monitor and review client progress, adjusting case plans as needed.
Requirements:
- Strong experience in case coordination within the Human Services sector.
- Demonstrated ability to conduct needs assessments and implement care plans.
- Proven experience working with individuals from culturally and linguistically diverse (CALD) backgrounds.
- Ability to manage clients with complex physical and mental health issues.
- Knowledge of relevant legislative and regulatory frameworks.
- Excellent written and verbal communication skills, with strong analytical abilities.
