We are seeking a skilled and compassionate Client Care Coordinator to join our team at My Home Care Group. In this role, you will be responsible for coordinating the care needs of our clients, working closely with our care teams to ensure seamless care delivery.

You will have the opportunity to develop strong relationships with our clients and their families, and work collaboratively with our multidisciplinary team to ensure that our clients receive the highest quality care possible.

About the Role:

  • Coordinate the care needs of clients, including developing and implementing care plans
  • Work closely with care teams to ensure seamless care delivery
  • Maintain accurate records and reports of client care activities
  • Provide exceptional customer service to clients and their families

Requirements:

  • Certificate in Individual Support or equivalent qualification
  • Current CPR and First Aid certificates
  • National Police Clearance or willingness to obtain
  • The right to work in Australia
