Job Title: Community Care Professional
Overview:
As a Community Care Professional, you will be responsible for leading the development of aged care practice frameworks and contributing to cross-sector practice improvement.
Review and create clinical policies, procedures, and manuals aligned with legislation and best practice. Conduct holistic assessments of current practices, identifying risk and opportunity for growth.
Drive audit processes, data analysis, and reporting to support a culture of accountability and quality. Deliver training, education, and coaching across the organisation to uplift capability.
Represent Practice Excellence internally and externally, championing safety, rights, and care outcomes. Participate in SIRS and quality indicator reporting with high attention to detail and compliance.
Key Responsibilities:

Requirements:
- Bachelor of Nursing or equivalent qualification + AHPRA Registration
- Current NDIS Worker Screening Clearance (paid) – or willingness to obtain
- Current Working with Children Check (paid) – or willingness to obtain
- Current Open, Unrestricted Driver's Licence
- Completion of NDIS Orientation Module: Quality, Safety & You
