Job Description:

We are seeking a dedicated Housing Support Specialist to join our team. This role will provide case management support to individuals in both community and residential settings, assisting them to achieve housing stability and improved social inclusion.

Key Responsibilities:

  • Establish relationships with individuals that encourage self-determination and participation
  • Provide case management support to individuals in accordance with established models and standards of practice
  • Complete thorough assessments, inclusive of needs, risks, and vulnerabilities
  • Ongoing assessment of the individual's presenting issues, vulnerabilities, and needs from initial intake to exit
  • Individual case planning in collaboration with the individual, including a range of interventions that address their needs or goals
  • Implementation of case plans with active individual participation
  • Assisting individuals to access support services and establish community supports, where needed
Required Skills and Qualifications:

To be successful in this role, you will have:

  • Tertiary qualification in Community Services, Social Work, or a related field
  • Relevant experience in a social service environment, including client assessment and case management
  • A national police record check and QLD Working with Children Check (as required)
  • Driver's Licence
  • First Aid Certificate
