Case Manager

Centrecare is seeking a highly skilled and compassionate Case Worker to join our team in the Goldfields region. As a Case Worker, you will play a critical role in supporting individuals transitioning from prison to community life.
Key Responsibilities- Provide high-quality re-entry services to clients exiting prisons, ensuring their successful transition to community life.
- Facilitate and run group sessions in a prison environment, promoting positive change and behavior.
- Support clients with home visits in the local Kalgoorlie area, building strong relationships and trust.
- Develop and implement effective case management plans, working closely with clients, their families, and external agencies.
- Participate in case management of clients, ensuring their needs are met and their goals are achieved.
- Recognised tertiary level qualifications in Psychology, Social Work, Social Sciences, Counselling, Health, or related disciplines.
- Capacity to work within an Outreach model of service delivery, with a focus on community-based services.
- Highly experienced in working positively with Aboriginal individuals, families, and their communities, especially in remote locations of WA.
- Experience in casework and supporting access to social and other services.
- Ability to be an effective member of a team or to work autonomously, with strong communication and interpersonal skills.
- National Police Clearance and a valid driver's licence.
