Job Description:
This role is responsible for ensuring that all residents receive the highest quality of clinical care. The successful candidate will have a strong background in nursing and experience in leadership roles, preferably in an aged care setting.
Key Responsibilities:
- Clinical Expertise: Provide clinical expertise and leadership to clinical and care service teams across the facility.
- Care Planning: Assist Care Manager to lead carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
- Assessment and Documentation: Responsible for managing all aspects of the assessment, documentation, and submission of AN-ACC requirements within the facility.
- Risk Management: Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
- Team Support: Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
- Admission Process: Support the admission of new residents to facilities for optimal care assessments and initial consultations with residents and families.
- Record Keeping: Ensure accurate information is recorded in accordance with all organisational and legislative requirements (AN-ACC, resident records, and staff time recording).
- Roster Management: Input and oversight into the facility team rosters and ensure optimal skill mix available for the delivery of care to residents.
Requirements:
- Qualifications: Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
- Experience: Minimum 1 year experience as a Care Coordinator or similar leadership position.
- Skills: Engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
- Computing Skills: High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
- Communication Skills: Excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
- NDIS Clearance: Have a valid NDIS Workers Screening Clearance (or ability to apply).
