We are seeking a dedicated professional to fill a Community Worker position. This role is ideal for someone passionate about making a positive impact in their community.

  • Supporting individuals and families with housing insecurity, mental health challenges, and social isolation
  • Providing advocacy, crisis response, and complex case coordination
  • Collaborating with internal and external professionals to deliver integrated care

This is a hands-on, client-facing role that requires strong communication and advocacy skills. If you have a minimum of 2 years of experience in community services, social work, or a related field, we encourage you to apply.

Key Responsibilities

  • Manage referrals and complete intake assessments to determine client needs
  • Develop individualised care plans, incorporating psychosocial and risk assessments
  • Coordinate a range of supports including NDIS, housing, health, and social services
  • Advocate on behalf of clients in meetings with external stakeholders
  • Maintain accurate records and documentation

Required Skills and Qualifications

  • Degree in Community Services, Social Work, or equivalent
  • Membership or licence from an accredited professional body
  • Strong communication and advocacy skills
  • Resilience, cultural sensitivity, and a trauma-informed approach
