Key Responsibilities
- Conduct comprehensive wellbeing assessments and develop individualised plans that clearly identify and address clients' goals.
- Partner with clients, families, and our multidisciplinary team to respond swiftly to changing needs.
- Manage client feedback, provide timely updates to the Management Team and other stakeholders, and handle various administrative tasks to ensure smooth operations.

Essential Criteria
- Relevant Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields
- Experience in client case management
- National Police check
- Driver's license
