Key Responsibilities
  • Conduct comprehensive wellbeing assessments and develop individualised plans that clearly identify and address clients' goals.
  • Partner with clients, families, and our multidisciplinary team to respond swiftly to changing needs.
  • Manage client feedback, provide timely updates to the Management Team and other stakeholders, and handle various administrative tasks to ensure smooth operations.

 

Requirements

We are seeking a committed and compassionate individual who is sensitive to the diverse needs of older people from various social, economic, and cultural backgrounds.

The ideal candidate will excel at building rapport with clients and their families, embodying qualities of kindness, enthusiasm, and energy.

Adaptability to rapid changes is essential, alongside an organised approach to managing your caseload.

Essential Criteria

  • Relevant Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields
  • Experience in client case management
  • National Police check
  • Driver's license
