This role involves delivering high-quality support services through needs assessments, case planning, referrals and ongoing review.

Key responsibilities include:

  • Providing intensive case management support to individuals and families with complex needs;
  • Ensuring the delivery of culturally responsive and evidence-based services;
  • Guiding, mentoring and training Case Coordinators and Cultural Support Workers;
  • Contributing to service improvement, compliance and collaboration with internal and external partners.

Successful candidates will be required to undergo probity checks including National Criminal History Record Check and any relevant stated based checks.

Requirements

To be successful in this role, you must have:

  • A recognised Degree qualification in Social Work, social sciences, psychology or mental health or a recognised diploma in a related field and working toward a degree qualification;
  • Proven experience in case management, working with individuals presenting with complex needs;
  • Ability to work autonomously while effectively managing competing priorities;
  • Skill in applying trauma-informed, strengths-based and culturally responsive practice;
  • Demonstrated leadership capability, including mentoring, coaching and training staff.
