Job Opportunity:

We are seeking a skilled and compassionate Support Services Coordinator to join our team. In this role, you will work closely with clients to develop personalized plans that promote independence and community participation.

About the Role:

This is an exciting opportunity to make a positive impact in the lives of Aboriginal and Torres Strait Islander individuals and their carers. As a Support Services Coordinator, you will be responsible for:

  • Developing and implementing individualized care plans that cater to the unique needs of each client.
  • Providing support and assistance to clients in various aspects of their lives, including personal care, health care, and hygiene.
  • Facilitating group activities and outings to promote socialization and community engagement.
  • Contribution to the development of evidence-based programs to meet the evolving needs of aged-care clients.

Requirements:

To be successful in this role, you will need:

  • A minimum Certificate in Aged Care or relevant qualification.
  • Current C Class Drivers Licence (QLD).
  • Current Disability Worker Screening Check (Yellow Card).
  • Current First Aid and CPR Certificate.

While experience working with Aboriginal and Torres Strait Islander people and communities is highly desirable, it is not mandatory. We encourage applications from diverse backgrounds and skill sets.
