
Care Coordination Specialist

We are seeking a part-time Coordinator of Supports on a 3-day per week basis, benchmarked at a Casework Manager Level.
Key Responsibilities- Evaluate options available to participants and assist in selecting preferred services.
- Negotiate services and costs with providers, develop service agreements and bookings, and allocate budget accordingly.
- Organise assessments to determine funding requirements, manage budgets, and inform plan managers about fund breakdowns.
- Connect participants with mainstream or community services, provide optionality of services, and assist participants in understanding services and agreements.
- Enhance participant ability to coordinate, self-direct, and manage supports, resolve issues, and change or terminate service agreements as needed.
- Strong knowledge and understanding of the National Disability Insurance Scheme and support coordination processes.
- At least 1 year of experience working with people with a disability or in a similar environment.
- Relevant experience in service quoting, understanding of NDIS price guide, and provision of support work for people with a disability.
- Demonstrated ability to identify, analyse, respond to, and manage risk, incidents, and complaints positively and proactively.
- Excellent negotiation, problem-solving, conflict resolution, time management, and organisational skills.
