Role Summary:

Murrumbidgee Local Health District is seeking a Discharge Patient Flow Concierge Social Worker to provide social work assessment and intervention to current inpatients across MLHD sites.

The successful candidate will play a key role as part of the multi-disciplinary team to support patients with accessing external services, build links with local transport agencies, and facilitate equitable access to services via virtual platforms.

  • Key responsibilities include providing social work assessment and intervention, supporting patients with accessing external services, and building relationships with local transport agencies and community service providers.

Service provision will predominately be via virtual platforms to facilitate equitable access to the service for all non-base hospital facilities.

This position includes weekend coverage and the opportunity to job share and exercise flexibility in selecting work hours that align with personal lifestyle and commitments.

About You

Our ideal candidate will demonstrate:

  • A tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
  • Ability to effectively prioritise and organise own work/caseload, including effective utilisation of virtual care to meet service demand.
  • Excellence in written, verbal, interpersonal and negotiation skills with a high level of problem solving and service evaluation skills.
  • A comprehensive approach to organisational goals, strategies and operations.
