About the Role

We are seeking an experienced Occupational Therapist to join our team as a Community Care Coordinator. In this role, you will work with patients to complete assessments, recommend equipment and home modifications, and manage referrals.

As a Care Coordinator, you will work with patients to monitor and coordinate their treatment plans, educate them about their condition, and connect them with their healthcare providers. You will also evaluate their progress and provide ongoing support.

About You
