Restorative Care Clinician Job Summary

Our team is seeking a skilled clinician to fill the role of Restorative Care Clinician.

This position involves providing high-quality restorative care to patients, promoting their independence, mobility, and overall well-being.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare providers.

  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans to address these needs
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Monitor patient progress and make adjustments to care plans as needed
  • Provide education and support to patients and their families on self-care and disease management

Key Responsibilities:

  1. Assess patients' physical and emotional needs to develop effective care plans
  2. Implement care plans that promote patient independence and mobility
  3. Collaborate with multidisciplinary teams to ensure comprehensive care
  4. Monitor patient progress and adjust care plans accordingly
  5. Provide education and support to patients and families on self-care and disease management

Requirements:

  • Registration as an Allied Health or Registered Nurse clinician
  • Experience working in a clinical setting, preferably in restorative care or a related field
  • Strong communication and interpersonal skills
  • Able to work independently and as part of a team
