Mental Health Case Manager Opportunity

We are seeking a Mental Health Case Manager to provide expert case management services to clients with complex mental health needs.

Your Key Responsibilities:

  • Conduct thorough assessments of client referrals and urgent risk assessments where necessary.
  • Manage a caseload of clients and provide specialized case management services.
  • Record accurate case notes and maintain up-to-date case management records.
  • Provide advice and support to non-clinical staff on assessment of client referrals.
  • Develop and implement short-term case management interventions and connect clients with ongoing support services.

Requirements for Success:

  • Bachelor's degree or higher in Nursing, Psychology, Occupational Therapy, or Social Work.
  • Minimum 2 years' experience as a case manager or in a clinical role.
  • Extensive experience working in mental health or community case management settings, particularly with clients with complex conditions.
  • Experience coordinating a caseload of high-risk clients.
  • Proficiency in triage, assessment, and case management via telephone.

The successful candidate must be an Australian Citizen and hold a current Police Check. Experience working with military or veteran mental health services, or supporting clients with substance abuse, mood disorders, or PTSD will be highly regarded.
