Your Key Responsibilities
- Assess and manage patients with chronic diseases, including developing personalized care plans
- Deliver health education and promote healthy lifestyles through counseling and support

- Maintain accurate records and collaborate with other healthcare professionals to ensure seamless patient care

- Current registration with AHPRA
