About the Role

Mercy Aged and Community Care Ltd is seeking a highly skilled and compassionate Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in coordinating the rostering and support of a team of Home Care Workers to ensure the right employee is rostered to provide the right service, to the right client, at the right time.

Key Responsibilities

  • Coordinate the rostering and support of Home Care Workers to ensure seamless delivery of services to clients.
  • Work closely with care advisors, referral agencies, and service providers to ensure effective communication and coordination.
  • Develop and maintain strong relationships with clients, families, and healthcare professionals to ensure high-quality care and support.
  • Monitor and evaluate the effectiveness of care plans and make recommendations for improvement.
  • Collaborate with the team to achieve service excellence and continuous improvement.

Requirements

  • Excellent time management, organisational, and communication skills.
  • A compassionate, flexible, and positive approach to effective patient care and management.
  • The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
  • A current NDIS Worker Screening Check (WSC) and COVID-19 vaccination certificate.
