The Role

As a Care Coordinator Specialist, you will play a pivotal role in delivering high-quality services through assessment, case management, and service monitoring for eligible clients. Reporting to the Clinical Care Manager of Home and Community Care Services NSW, your key responsibilities include:

  • Candidate with a Certificate IV in Individual Support and experience in case management or an Enrolled nurse qualification are encouraged to apply.
  • You will need to balance client expectations with available resources, effectively communicating these to clients.
  • Develop and review individualised care plans based on client assessments and preferences.
  • Excellent communication, interpersonal, and negotiation abilities are essential.
  • Maintain accurate records and documentation in line with organisational policies and government funded guidelines.
  • A collaborative approach to working as part of a team is required.
  • Knowledge of Commonwealth Support Programme (CHSP) is highly valued.
  • Demonstrated experience in assessment, referral, and coordination of human services is necessary.
  • An unrestricted driver's license is also required.

Key Requirements

  • You must be eligible to work in Australia and willing to undergo a criminal record check and NDIS check as required by the Aged Care Act 1997.
