The Role We Are Hiring ForWe are seeking an experienced Aftercare Lead to join our team. This role will be responsible for coordinating and integrating aftercare services across the district, working closely with stakeholders to ensure consistent, high-quality care for individuals following a suicidal crisis.
- This position will provide expert advice, support governance and evaluation processes, and help embed best practice approaches to improve referral pathways in a multidisciplinary setting.
- You will work collaboratively with government agencies, health networks, and community service providers to implement or enhance mental health services.
This role requires strong leadership skills, strategic thinking, and effective communication. You will have:
- A tertiary qualification in a relevant field (e.g., social work, psychology, nursing) or equivalent experience in mental health and suicide prevention.
- Significant experience in coordinating stakeholder engagement, supporting referral pathways, and contributing to data reporting and quality initiatives.
