Community Care Specialist

We are seeking a Personal Carer to provide care to clients living in their own homes, supporting their independence and improving their quality of life.
- Key Responsibilities:
- Deliver personalized care to clients, focusing on their individual needs and goals.
- Complete scheduled visits, engaging with clients in their homes and supporting them with daily activities.
- Provide in-home personal care, including assistance with daily living tasks such as showering, meal preparation, house cleaning, and laundry.
- Support clients in accordance with their individualized Care Plans.
- Facilitate community-based recreational and social activities that promote clients' well-being and engagement.
- Assist clients with transportation, shopping, and appointments as needed.
- Certificate III Aged Care or Individual Support qualification - Essential.
- Current Australian Driver's Licence (Green Provisional minimum) - Essential.
- Ownership of a roadworthy and insured vehicle for work purposes - Essential.
