Case Manager

Job Description:
As a Case Manager, you will be responsible for undertaking comprehensive assessments to identify the suitability of kinship care placements. You will consider risk factors, strengths, and weaknesses of the placement, as well as individual child needs.
You will support children to settle within their placements and provide guidance to kinship carers on how to build their capacity to respond to the child's safety, stability, and developmental needs.
Key Responsibilities:
- Conduct thorough assessments to determine the suitability of kinship care placements
- Identify risk factors, strengths, and weaknesses of each placement
- Develop recommendations for Child Protection to inform their decisions on kinship care placements
- Provide ongoing support to kinship carers to help them meet the needs of the children in their care
Requirements:
- Qualifications in social work or a related field
- Experience working with vulnerable populations, including children in out-of-home care
- Strong assessment and report-writing skills
- Ability to work collaboratively with other professionals, including Child Protection workers and kinship carers
