Job Description:

Key Responsibilities
  • Work with clients, carers, and families to identify clinical goals
  • Develop and maintain clinical care plans
  • Work within current Calvary policy and procedures and WHS guidelines
  • Coordinate and organise the provision of nursing care including clinical comprehensive assessments
  • Work with HCP case managers and liaise with VHC program/DVA community nursing and other services
Requirements
  • AHPRA Registration
  • Recent nursing/clinical experience in the last 5 years or evidence of a refresher course
  • Effective communication, interpersonal and organisational skills
  • Initiative and the desire to continually improve client care
  • Capability to prioritise tasks and achieve goals within set timeframes
  • Candidates with an understanding of the community nursing sector - Home Care Packages, Department of Veteran Affairs and Commonwealth Home Support Program will be considered favourably
  • A desire to join a team committed to the care of others in a mission-based organisation
