Community Care Specialist

We are seeking a skilled professional to support elderly individuals in their homes, promoting independence and dignity.
As a Community Case Manager, you will assess customer needs, develop personalized care plans, and manage budgets to ensure each customer's unique requirements are met.
Your role will involve collaborating with Nurses and Allied Health professionals to deliver comprehensive care, while maintaining a person-centered approach.
You will work with customers in the South West suburbs of Sydney, including Liverpool, Smithfield, Chullora, and Leura.
About You- Previous experience in community-based case management is preferred.
- Experience with Home Care Packages is desirable.
- Current driver's license and a reliable vehicle with insurance.
- Strong communication skills, both verbal and written.
- Demonstrated empathy and compassion.
- Excellent problem-solving abilities and critical thinking skills.
- Good organizational and time management skills.
- Cultural competence and the ability to work with diverse populations.
- Advocacy skills to ensure clients' needs are met.
- Collaboration and teamwork skills to work effectively with interdisciplinary teams.
