Position Overview:
Part-time role (4 days) - Flexible travel required across various locations.
As a Dementia Care Specialist, you will be instrumental in providing high-quality, person-centered support, guidance, and advocacy for individuals living with dementia and their families. Your role will not only focus on client well-being but also on raising awareness and educating the community about dementia care. This position demands empathy, excellent communication skills, and a profound understanding of dementia-related issues.
Key Responsibilities:
- Client-Centered Support: Ensure clients and their caregivers are treated with respect and compassion, delivering services that allow them to live their lives according to their unique needs.
- Case Management: Oversee case management in accordance with organizational policies, ensuring clients receive the necessary support.
- Advocacy and Support: Offer evidence-based advice and advocacy during home visits or group settings, assisting individuals living with dementia and their caregivers in navigating challenges.
- Community Education: Organize educational sessions and events to enhance community awareness of dementia, supporting families and local networks.
- Exceptional Customer Service: Promote a culture of outstanding customer service within the team, focusing on the needs and preferences of clients and their caregivers.
- Assessments and Care Planning: Conduct and review assessments and care plans promptly to ensure effective and efficient service delivery.
- Financial Management: Ensure services operate within budgetary constraints, managing financial resources effectively.
- Administrative Duties: Manage all administrative responsibilities, including service confirmations, compliance, scheduling, and client records, with accuracy and timeliness.
- Stakeholder Engagement: Establish and maintain positive relationships with all internal and external stakeholders, ensuring effective communication and confidentiality.
Required Skills and Qualifications:
- Diploma in Community Services/Dementia Care or equivalent qualification is essential.
- Comprehensive understanding of dementia, its progression, and its effects on individuals and their caregivers.
- Demonstrated experience working with older adults, particularly those living with dementia and their families.
- Excellent communication skills, both written and verbal, with strong presentation capabilities.
- Proficient in Microsoft Office and client database systems.
- Valid driver's license and access to a vehicle.
