Home Care Case Manager

  • A key role in our Home Care team is the Case Manager.

The primary function of a Case Manager is to oversee the provision of home-based case management and care services.

  • This entails planning, coordinating and ensuring that all care services are aligned with best practice and meet the needs of clients.
  • In this capacity, you will be working closely with families, health providers and care workers to deliver high-quality support and ensure seamless service delivery.
About This Role

This is a challenging and rewarding role where no two days are the same.

  • As a Case Manager, your responsibilities will include mentoring staff, reviewing services, solving rostering puzzles and working collaboratively with colleagues to achieve shared goals.
  • Building strong relationships with clients and their circles of care is essential to delivering effective support.

The ideal candidate has:

  • A certificate III in Individual Support (Ageing) or equivalent qualification.
  • Cert IV is highly desirable, but not essential as long as you are willing to obtain it.
  • Completion of specified units of competency including assisting clients with medication, recognising healthy body systems and providing individualised support.
  • At least one year of experience leading a care or support team.
  • A current driver's license and insured vehicle.
  • Current First Aid and CPR certification.
  • Knowledge of Home Care Packages.
