We are seeking a dedicated and motivated individual to fill the role of Aged Care Integration Officer. This position will play a crucial part in supporting the strategic objectives of our organisation, focusing on primary healthcare and community wellbeing.

The successful candidate will have a strong passion for improving the lives of older people and those requiring palliative care. Key responsibilities include developing and coordinating activities related to older people's health, aged care, and palliative care.

This role involves implementing The Aged Care On-site Pharmacist Measure in residential aged care homes and coordinating engagement and support for the Allied Health sector in our region.

Based in one of our offices located in Dubbo, Orange, Bathurst or Broken Hill, this role offers a unique opportunity to make a meaningful impact in the lives of others.
