Comprehensive Aged Care Assessment Role

Aged care assessment involves understanding the needs of older people to provide appropriate support and services.

About the Role

We are seeking an experienced professional to conduct comprehensive assessments of older people's needs, working across a range of aged care settings.

Key Responsibilities

  • Conduct thorough assessments of older people's needs, using detailed knowledge of relevant policies and programs.
  • Formulate and implement effective care plans in collaboration with relevant stakeholders, including healthcare professionals and family members.
  • Utilize technology to support assessment and planning, such as electronic patient record systems and e-Learning sites.
  • Communicate effectively with older people, their families, and other stakeholders, providing clear and timely information about assessment outcomes and care plans.

Requirements

  • Relevant tertiary qualifications, such as nursing, occupational therapy, social work, or physiotherapy, with full registration or eligibility for membership of the relevant Australian peak professional body.
  • Proven experience working in aged care settings, with a focus on assessment and care planning.
