Occupational Therapist

We are seeking an experienced and dedicated occupational therapist to join our team as a community support specialist.
About the Role:- This is an exciting opportunity to work with adults in their own homes, promoting independence and wellbeing.
In this role, you will assess the needs of adults with physical or cognitive challenges and provide tailored support to enable them to live safely and independently.
Responsibilities:- Assess clients' needs and develop personalized care plans to promote independence and wellbeing.
- Recommend equipment and adaptations to support daily living activities.
- Work collaboratively with health and social care professionals to deliver joined-up support.
- Conduct home visits to assess clients' environments and provide guidance on safety and accessibility.
- Keep accurate records and contribute to care planning processes.
- Support safeguarding and promote wellbeing in the community.
- A qualified occupational therapist registered with the HCPC.
