Job Summary:

The Health Care Aide provides direct client care and environmental support within established standards of care, policies and procedures.

Responsibilities include:

  • Supporting Clients in Daily Life, we strive to maintain the self-respect, dignity, and independence of clients by encouraging and allowing clients to care for themselves within the limits of their abilities as described in the client's record.
  • We communicate client changes to the team leader.
  • Care documentation is maintained according to policy.
  • The Health Care Aide participates in client care conferences (as required) and in the development of individual care plans.
  • We provide or assist clients with Personal Hygiene as per care plan.
  • We provide or assist clients with Elimination as per care plan.
  • We provide or assist clients with Mobility as per care plan.
  • We provide or assist clients with Observation and Measurements as per care plan.
  • We ensure necessary safety measures are provided or assisted.
  • We use Validation and Orientation Techniques to assist clients.
  • A clean environment is maintained.
  • Unit supplies are ordered and stocked.
  • We encourage and assist clients' participation in activity programs.
  • Additional duties may be assigned.
