Nurse Practitioner

We are seeking a dedicated and compassionate Nurse Practitioner to join our team at St Vincent's Health Australia. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our care services.
Key Responsibilities- Provide expert nursing care to patients, including assessment, planning, implementation, and evaluation of care.
- Collaborate with multidisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
- Contribute to the development and implementation of policies and procedures that promote excellence in patient care.
- Participate in quality improvement initiatives to ensure that our care services meet the highest standards.
- Provide education and support to patients, families, and caregivers to promote health and well-being.
- Nursing Registration with the Australian Health Practitioner Regulation Agency is essential.
- Clinical nursing practice experience in an aged care facility is preferred.
- Experience with NDIS and Huntington's Disease is highly regarded.
- Leadership skills with the ability to lead a team by example.
- High level of customer service skills.
- Strong attention to detail and excellent communication skills.
- Ability to follow directives as per care plan.
- Excellent record keeping skills with experience using Autumn Care (or similar) desirable.
- Understanding of Aged Care and resident rights and responsibilities.
- National Police Certificate.
- NDIS Worker Screening Check is a requirement of this role.
