Job Description

About the Role


We are seeking a compassionate and experienced Registered Nurse to join our team as an Aged Care Coordinator. In this role, you will be responsible for providing direct patient care, including administering medications and interventions, and coordinating with a multidisciplinary team to develop and implement bespoke care plans.



  • Monitor and document resident health status and progress, making referrals as needed;
  • Provide guidance and support to our team of care workers;
  • Cultivate strong relationships with residents, their families, and the broader care team;
  • Maintain up-to-date knowledge of clinical procedures and best practices in aged care;
  • Contribute to continuous quality improvement initiatives within the facility.

Requirements

To be successful in this role, you will need:



  • Current AHPRA registration;
  • Clinical knowledge and experience in aged care;
  • A National Police Check or ability to obtain;
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
