We are seeking an experienced Care Team Leader to join our team in Kalgoorlie. In this challenging and rewarding role, you will have the opportunity to lead a team of care staff and make a positive impact on the lives of clients.

Key Responsibilities

The successful candidate will have a strong background in aged care and be able to demonstrate the following skills and qualifications:

  • Leadership and management experience within a community environment.
  • Demonstrated knowledge and experience undertaking care management activities within a Support at Home program.
  • Well-developed communication and interpersonal skills to develop effective relationships with clients and staff.
  • Decision making and problem-solving skills to innovatively manage service delivery.

Additional responsibilities include:

  • Coordinating and leading all care management activities for clients within the Support at Home Program.
  • Conducting initial and ongoing strength-based assessments to review client care requirements and adjust accordingly.
  • Developing care plans that ensure alignment with client needs and goals, Aged Care Quality Standards, and funding requirements.
