Job Description

This role is located within the APY Lands in South Australia and will see you working within Aboriginal communities, including Pukatja, Pipalyatjara, Mimili, Kaltjiti, Iwantja and Amata.

The successful candidate will be responsible for monitoring and reviewing current systems and processes to improve internal and external procedures for home care. This will involve coordinating and analysing data for monthly reporting requirements, ensuring team adherence to funded services within clients' plans, optimising and reviewing services.

You will also provide quality client service, interacting with clients, answering client enquiries and effectively handling client complaints. Additionally, you will develop trusting, positive and professional relationships with clients and service providers both internal and external.

Key Responsibilities:
  • Improve internal and external home care procedures.
  • Coordinate and analyse data for monthly reports.
  • Ensure team adherence to client-funded services.
  • Provide quality client service and handle complaints.
  • Develop strong relationships with clients and service providers.
Requirements:
  • A minimum of an AQF Certificate III in Individual Support.
  • At least one year of relevant work experience (post-qualification) undertaken in the last 3 years.
  • Previous work in a Team Leader or similar role with preference within a Home Care setting.
  • Previous work in a team within Community Services, such as Aged Care, Disability, Mental Health, Housing or a similar industry.
  • Culturally sensitive practice when working with Aboriginal and Torres Strait Islander people and people from culturally and linguistically diverse backgrounds.
  • Prior use of Microsoft Office suite and other online client reporting systems.
  • Ability to respond and adapt to competing priorities while working unsupervised.
  • Empathy with aged people, their families, and personal carers.
