Job Title: Community Physiotherapist

We are seeking a skilled Physiotherapist to join our multidisciplinary team in providing exceptional quality care to our clients in the Aged Care and Disability sectors. As a Community Physiotherapist, you will work in a dynamic and supportive environment, delivering in-home care and building relationships with participants and their families.

Key Responsibilities:
  • Conduct high-quality Physiotherapy assessments and interventions for consumers in a professional, confidential, and culturally safe manner.
  • Complete consumer-focused Physiotherapy assessments to manage a range of musculoskeletal, neurological, and cardio-respiratory disorders.
  • Ensure integrated service delivery and follow-up, coordinating and communicating as required.
  • Incorporate a holistic approach to healthcare, including health promotion activities tailored to consumers' priorities and requirements.
  • Ensure accurate and timely recording of information in our management system, adhering to Aged Care Quality Standards and company policies.
  • Provide manual handling and education training for carers and family members.
  • Consult with site staff, doctors, consumers, family, and other care professionals to develop consumer care plans, goals, and interventions.
  • Regularly review consumers' outcomes and make necessary changes to align with consumer goals.

  • Excellent communication and organizational skills.
  • Current Working With Children Check.
  • Valid Australian working rights.
  • Full Registration with AHPRA.
