About the Role

As a VMCH Case Manager, you will play a pivotal role in delivering high-quality care and support to consumers with diverse and complex needs. Your responsibilities will include developing tailored care plans, managing consumer budgets, and ensuring compliance with Aged Care Reforms and Quality Standards, and the HCP legislative framework. Your expertise in Case Management, customer service, and autonomy will be essential in delivering exceptional care and support.

About You

What you will bring to the role:

  • A health-related tertiary qualification or a related field.
  • Previous experience as a care manager developing care plans and managing consumer budgets.
  • An understanding of Aged Care Reforms and Quality Standards.
  • Proficiency in MS Office and Client Management applications.
  • Aged Care knowledge.
  • A customer service focus.
  • A willingness to travel.
  • A driver's license and police check required.

Key Responsibilities

As a VMCH Case Manager, your key responsibilities will include:

  • Developing and implementing tailored care plans that meet the unique needs of consumers.
  • Managing consumer budgets and ensuring compliance with Aged Care Reforms and Quality Standards.
  • Collaborating with various stakeholders, including healthcare professionals, family members, and carers.
  • Providing exceptional customer service and support to consumers and their families.
  • Staying up-to-date with industry developments and best practices in Case Management.
