About the Job

We are seeking a compassionate and skilled Nurse to provide community nursing services across Toowoomba.

">

The successful candidate will travel to clients' homes, conducting comprehensive assessments and case management while applying clinical knowledge to implement planned care.

Key Responsibilities

">
  • ">
  • Conduct accurate and timely assessments, representing each client's current status and requirements.">
  • Develop holistic and effective care plans from assessments.">
  • Ensure compliance with ACQSC Quality Standards and the Aged Care Act through proper documentation.">
  • Communicate effectively with clients, families, and stakeholders in ongoing care delivery development and review.">
  • Regularly review clients' progress against their care plan, modifying as needed due to changed care needs.">
  • Liaise with healthcare providers involved in client care plans.">
  • Deliver care safely and competently.">
  • Evaluate evidence-based resources and knowledge for best practice.">
  • Document care provided according to organisational guidelines and client management systems.">
  • Coordinate delivery of care by staff (ENs, HCWs, support staff) and delegate tasks appropriately.">
  • Administer medications in accordance with Drug Health and Poisons Act and prescribed orders.">
  • Participate in case conferences with clients, families, and relevant service providers.">

Requirements

  • ">
  • Current National Police Certificate">
  • Current Drivers Licence and reliable vehicle">
  • Current CPR Certification">
  • Degree in Nursing or equivalent qualifications and experience to Registered Nurse level">
  • Current AHPRA registration and recent practice evidence">
  • Experience working as a Registered Nurse in community aged care">
  • Ability to coordinate quality nursing care within a risk management framework.">
  • Effective time management and problem-solving skills">
  • Ability to build strong relationships and maintain open communication within a multidisciplinary team.">
  • Previous aged care experience is highly desirable.">
