Nurse Care Manager

We are seeking a highly skilled and compassionate Registered Nurse to join our team at The Shoreline, a luxury aged care facility on the Mid North Coast of NSW. As a Clinical Lead Nurse, you will be responsible for managing all aspects of clinical care for our residents, including dementia, falls management, wound care, continence management, and palliative care.
Key Responsibilities- Provide high-quality clinical care to our residents, ensuring their physical, emotional, and social needs are met.
- Develop and implement individualized care plans, working closely with our team of Enrolled Nurses, Companions, and Allied Healthcare professionals.
- Conduct regular assessments and reviews to ensure the best possible outcomes for our residents.
- Manage medications and ensure accurate documentation.
- Lead and participate in case conferences with residents, families, and healthcare professionals.
- Monitor and analyze clinical indicators, making recommendations to improve care quality.
- Deliver care in accordance with the needs, goals, and preferences of each resident.
- Manage admissions, leave, and discharge processes.

Requirements- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated experience in residential aged care nursing, with a focus on clinical assessments, care planning, and medication management.
- Ability to work independently and professionally with limited supervision.
- Strong organizational skills and ability to prioritize tasks effectively.
- Commitment to ongoing professional development and a willingness to learn and adapt to new situations.
