Exceptional Care Professional

Delivering person-centred care to individuals with varying levels of need is at the heart of this role. Daily routines are tailored to each client, ensuring their unique requirements are met with compassion and understanding.

Responsibilities:

  • Support clients with daily activities such as personal care, meal preparation and medication management
  • Develop and implement individualised plans to promote independence and quality of life
  • Communicate effectively with clients, families and healthcare professionals to ensure seamless care coordination
  • Maintain accurate records and reports to track client progress and provide valuable insights

Requirements:

  • Certificate III in Disability or higher, or equivalent experience
  • Current Working with Children Check
  • First Aid/CPR certificates
  • Reliable vehicle and valid driver's license
  • Punctuality, reliability and flexibility
  • Good written and verbal communication skills
  • Empathetic and proactive approach to client care
